Document Submission Methods

How to Submit Required Documentation for Admission

ACT, ACCUPLACER, or SAT

Official score reports may be sent to the UACCM Admissions Office in person, by the Admission Document Submission form, by mail, or by fax. Keep in mind that scores are only valid for five years.

Transfer students who have successfully completed math, English, and/or reading courses may not be required to submit placement test scores. All first-time entering freshman and concurrently enrolled high school students are required to submit placement test scores.

College Transcripts

Official college transcripts may be sent to the UACCM Admissions Office by mail directly from the previous institution, through official electronic means if the previous institution has that capability, or in person inside a sealed envelope from the previous institution. Faxed or emailed transcripts will not be considered official.

Concurrent Enrollment Form

The Concurrent Enrollment Agreement may be completed online. It is to be completed by the student and parent/guardian.

Felony Committee Approval

Documents related to the Felony Conviction Review Process may be sent to the Vice Chancellor for Student Services. UACCM strives to provide a safe campus and learning environment. Individuals who indicate on their Application for Admissions that they have previously been convicted of a felony will receive a letter detailing documentation that is to be submitted to the committee.

  1. A letter from you addressed to the review committee explaining the details of the crime (what happened?) and why you should be allowed to attend UACCM.
  2. At least one letter of recommendation, preferably from your employer, probation/parole officer, counselor, etc.

The existence of a felony conviction does not mean that you will be denied admission to UACCM, but rather this process allows for a committee to review the specifics of your conviction and make a determination as to whether your enrollment as a student is consistent with our desire for a safe campus.

GED® Scores

Official General Education Development (GED®) scores may be sent to UACCM Admissions Office in person, by the Admission Document Submission form, mailed, or faxed. If you do not have a copy of your GED® scores, you may be able to order it online. Arkansas GED® graduates may order a duplicate transcript here.

Transfer students who have successfully completed 15 college-level credit hours are not required to submit a high school transcript/GED® scores. All first-time entering freshmen must submit a high school transcript/GED® score regardless of the number of concurrent credit hours completed.

High school permit to register form

The High School Permit to Register Form may be completed online.  It is to be completed by the high school or home school official authorizing student enrollment in the concurrent program at UACCM.

High School Transcript

Official high school transcripts may be sent to the UACCM Admissions Office in person, by mail, or by official electronic means.

Immunization Record

Immunization records may be sent to the UACCM Admissions Office in person, by the Admission Document Submission form, by mail, or by fax.

Arkansas State Law requires all students to provide proof of immunization for measles, mumps, and rubella. (Two measles immunizations, one mumps immunization, and one rubella immunization; MMR.) If shot records are under a previous last name, please indicate your current last name on the document. Applicants seeking an exemption for any reason must apply with Arkansas Department of Health. Applicants born prior to January 1, 1957 may submit proof of date of birth in lieu of immunization records.

International Student Documentation

International student admission requirements, except transcripts, may be sent to the UACCM Admissions Office in person, by the Admission Document Submission form, by mail, or by fax. Visiting Student Form

The Visiting Student Status Statement of Intent may be completed online.
